Office Policies
Kate O’Hara Park is committed to the goal of providing the best possible therapeutic care. The information on this page is presented with the belief that your clear understanding of our policies is an important part of maintaining a helpful and satisfying relationship.
Confidentiality
We understand the importance of the privacy of information you may share here. Usually we will not release information to others without you written consent. However, there are some exceptions to this general rule. If you reveal information about past or current abuse of a minor, elderly person, or disabled individual we must report this to an appropriate state agency.
Referral Source
We would prefer to have the opportunity to discuss your care with any health care provider that may have referred you in order to coordinate your care. This is your decision, and we will respect your wishes if you want to maintain privacy. Please inform me if you wish to limit my contact with other members of the health care team.
Emergencies
Discussions of problems in daily living are best reserved for the regular therapy sessions. However, you may experience a crisis that leads you to believe that prompt action is necessary to prevent harmful or impulsive behavior. Examples of this might include persistent thoughts of suicide, urges to harm others or a sense that your thoughts or behaviors are out of control. Please call 911 in such situations or visit he emergency room of a hospital.

Prepare for your appointment
The forms below (BIAS and consent forms) can be downloaded, printed, and brought into the office to further quicken your process. The insurance form can be filled out electronically before printing and signing/dating. Thank you.
